Please be advised that a quote of eligibility and benefits is not a guarantee of payment. All benefits and claim payments are subject to eligibility, medical necessity and the terms, conditions, limitations, exclusions and payment provisions of the patient’s health benefit plan at the time the services are rendered. Benefit amounts are usually not based on billed charges and might be significantly less than billed charges.

Your health insurance company will only pay for services that it determines to be “reasonable and necessary.” Our office will make every effort to bill your insurance in a timely manner. If your carrier determines that a particular service is not reasonable and necessary, or that a particular service is not covered under the plan, your insurer will deny payment for that service and it will become your responsibility.

We recommend you to be familiar with and verify your benefits with your insurance company prior to your services at Progressing through Therapy, PLLC. Please be aware, that even then, it is still not a guarantee of benefits or payment.

As a courtesy, we attempt to verify health insurance benefits prior to initial visit. While we try to avoid situations in which insurance coverage is expected but later denied, we cannot guarantee the services provided will be reimbursed. It is up to you to know your and/or your child’s insurance coverage, including knowledge of co-payment amounts, yearly deductibles, and co-insurance amounts. We cannot be responsible for verifying this information for you.

As a courtesy, we file your insurance for you, but must have your full insurance information, including secondary insurance, in order to do so. Payment for deductible charges, co-payments, and co-insurance charges is expected at time of service.

If insurance payment is not received with ninety (90) days after a claim is filed, the client is then responsible for payment of the total amount due. It is your responsibility to follow-up with your insurance company for delayed payments or other concerns.
