In the event a client requires paperwork for short term disability, long term disability, family and medical leave, school assignments, etc., clients are aware that they [the client] will have satisfied the following requirements:

• Client will have been attending sessions for a minimum of 12 months or 48 sessions. 
• Client is utilizing skills learned in therapy
• Client will agree to seek consultation with a psychologist or a psychiatrist, in addition to seeking consultation with primary care physician
• In the event, client satisfies requirements for paperwork completion, there is a $25.00 fee payable prior to completion of paperwork.
• Paperwork turnaround time is 15 business days
